Transitions Program: Assists youth 19-21 in linking with their community for their transition from high school to adult life. Individualized funding (purchase of services: Supports to individuals with developmental disabilities to participate in their communities. Dual Diagnosis Transitional Rehabilitation Housing: Residential supports for max 2 year period where individuals are provided education training and specific supports to transition them to appropriate community placement.
