MCEWEN MANOR PROGRAM SERVICES 39 RESIDENTS AND PROVIDES LONG TERM HOUSING IMPROVED QUALITY OF LIFE CONCURRENT DISORDER PROGRAMMING AND LESS INVOLVEMENT WITH ACUTE CARE SERVICES PEARL MANOR - ACQUIRED BRAIN INJURY COMMUNITY RESIDENTIAL PROGRAM - PROVIDES REHABILITATION SERVICES TO PERSONS WITH ABI IN A SUPPORTED APARTMENT WITH A CAPACITY OF UP TO 10 CLIENTS. ALSO PROVIDES SUPPORT TO 15 PERSONS IN THEIR OWN RESIDENCE. PHOENIX HOMES PROGRAM - HOMES IS A HOUSING FIRST MODEL PROGRAM WHICH AIMS TO PROVIDE IMMEDIATE ACCESS TO HOUSING FOR INDIVIDUALS WHO ARE CHRONICALLY HOMELESS ALONG WITH INTENSIVE SUPPORT SERVICES ONCE IN HOUSING. PHOENIX INTENSIVE PROGRAM SERVICES (PIPS) - GEARED TOWARD CLIENTS LIVING IN THE COMMUNITY IN VARIOUS LOCATIONS (ON THEIR OWN SALVATION ARMY APPROVED HOMES) WHO FREQUENTLY OR CONSISTENTLY HAVE DIFFICULTY MAINTAINING THEIR HOUSEHOLD; MANAGING THEIR FINANCES MEDICATION HYGIENE AND PERSONAL HEALTH; AND WHO MAY HAVE LEGAL ISSUES DIFFICULTY AVOIDING PUTTING SELF AT RISK AND PROBLEMATIC SUBSTANCE USE & POTENTIAL FOR HOMELESSNESS THE EATON INTENSIVE PROGRAM - PROVIDES 24-HOUR STAFF SUPPORT ON SITE IN AN APARTMENT STYLE RESIDENCE FOR UP TO 10 INDIVIDUALS. THE GOAL OF THE PROGRAM IS TO BE A TRANSITION STEP TO LESS INTENSIVE SERVICES THAT EXIST IN THE COMMUNITY. THE PROGRAM IS GEARED FOR THOSE INDIVIDUALS WHO ARE IN AN INPATIENT FACILITY OR SHNB AND ARE READY TO TRANSITION INTO THE COMMUNITY BUT STILL REQUIRE A HIGH DEGREE OF SUPPORT. THE POTENTIAL FOR INDEPENDENT LIVING MUST EXIST. THE PROGRAM MAY ALSO BE USED AS AN ALTERNATIVE TO HOSPITALIZATION FOR INDIVIDUALS LIVING IN THE COMMUNITY WHO ARE STRUGGLING WITH MAINTAINING THEIR PSYCHIATRIC STABILITY.
