THESE CHILDREN MAY STAY LONGER THAN SEVENTY-TWO HOURS AS THEY AWAIT TO TRANSFER OR KINSHIP CARE PLACEMENT. FOLLOW UP PROGRAM SUPPORT IS SCHEDULED WITH THE CAREGIVERS FOR AT LEAST ONE FOLLOW-UP CONTACT AFTER DISCHARGE IN THE FAMILY’S HOME. IF NEEDED FOR FURTHER SUPPORT A FOLLOW UP WORKER WILL ENGAGE CAREGIVERS IN ADDITIONAL HOME-BASED SUPPORT TO WORK ON GOALS THAT WILL REDUCE RISK FACTOR AND IMPROVE THE FAMILIES FUNCTIONING AND WELLBEING.
