One Care Home and Community Support Services provides Home and Community Support services for seniors and those with health challenges allowing them to live independently at home within a network of support. Our programs and services include: • Personal and In-home Support Services: Provides assistance with daily living activities such as bathing dressing as well as with household tasks to help individuals maintain their independence at home. • Respite Care: In-home and overnight support in several locations providing relief for caregivers allowing them to take a break while ensuring their loved ones receive care. • Adult Day Programs: Provide caregivers with a break while their loved one participates in activities that promote health and wellbeing. A special stroke rehabilitation program is available for those recovering from a stroke • Assisted Living: Provides care in designated locations for those who require 24-hour support in their own homes. • Community Support Programs - Offers a range of programs designed to help individuals be healthy at home and engage with their community including Meals on Wheels transportation dining and social activities health and wellness programs foot care and volunteer companionship services. • Care Planning Services: Provides assessment and assists individuals in navigating healthcare resources and developing personalized care plans to meet their unique needs. • Coordinated Transition support from hospital to home: Helps clients with transportation and support to ensure a safe discharge and successful recovery.
