QUEST COMMUNITY HEALTH CENTRE

Quest CHC provides primary health care services to those experiencing social economic and cultural barriers. Our team includes GPs NPs RNs RPNs social workers dietitians therapists outreach workers and a full dental team. Our health promotion includes free groups offering chronic disease management mental health and staying healthy. Groups for Quest CHC clients include Skills to Cope Craving Change and Healthy Choices to Recovery. Other groups are available to the community (e.g. Smoking Cessation Chronic Disease Self-Management Trans Youth Group). We deliver culturally competent 2SLGBTQIA services to individuals across their lifespan. The Urgent Service Access Team connects clients to ongoing primary health care mental health/addictions services and other social services. Quest provides primary care and health teaching to Niagara's Seasonal Agricultural Workers. Our volunteer chiropractors provide complete assessments and treatment plans for pain management and lifestyle modifications. Our Homeless Emergency Liaison and Peer Support Program(HELPS Niagara) has Peer Support Navigators with lived experience of homelessness providing nonclinical support advocacy and referrals for homeless persons attending the Emergency Departments at Niagara Health. The PreP Program provides PreP medication stopping the spread of HIV protecting those most at risk. The Ontario Seniors Dental Care Program provides free routine dental care for eligible low income seniors. Quest provides therapeutic and outreach support to at risk youth.
