THEY PROVIDE A FULL RANGE OF HEALTH CARE SERVICES TO THE COMMUNITY OF LEEDS & GRENVILLE. PROGRAMS PROVIDED INCLUDE HEALTH PROMOTION DISEASE PREVENTION DIABETES EDUCATION CHRONIC DISEASE MANAGEMENT AND COMPREHENSIVE PRIMARY HEALTHCARE SERVICES.
