THE HOSPICE IS WORKING WITH COMMUNITY AGENCIES AND THE FORMAL MEDICAL SYSTEM TO DEVELOP PROGRAMS AND SERVICES TO ADDRESS THESE GAPS. TO SUMMARIZE THE MANDATE OF THE OUTREACH PROGRAM IS TO SUPPORT BETTER THOSE LIVING WITH A LIFE-LIMITING DISEASE IN THE SETTING OF THEIR CHOICE (I.E. HOME HOSPICE ETC.)
