COMMUNITY-BASED SERVICES INCLUDE OUR FOOD SECURITY-FOCUSED SEED PROGRAM PARENT OUTREACH WORKER SERVICES WITHIN PRIORITY NEIGHBOURHOODS OUR PEER-LED BREASTFEEDING PROGRAMMING CHRONIC DISEASE SELF-MANAGEMENT PROGRAMS AND PEER-LEADER PROGRAMS AS WELL AS OUT STREET OUTREACH SERVICES( INCLUDING SPECIALIZED OUTREACH SERVICES AND WELCOMING STREETS ALONG WITH OUTREACH NURSING INTEGRATED WITHIN THE SHELTER SYSTEM). THE GUELPH CHC IS ALSO A SERVICE DELIVERY PARTNER FOR EARLYON SERVICES WHICH INCLUDE DROP-IN PLAY-BASED LEARNING OPPORTUNITIES FOCUSING ON EARLY CHILDHOOD DEVELOPMENT AND CAREGIVER SUPPORTS
